database issue with creation of records

It seems that I can only add one instance (record) for a phase, project,
contact, etc.

That is to say, if I add a project, "myproject", and save it, everything
works fine.  However, when I attempt to add a second project, Achievo only
provides me with the option of modifying the original.

It seems that the ID field in the database is not being stored/retrieved
correctly.

If anyone wants to take a look at this, I'll give them access to my
installation.  I am running FreeBSD 5.3/R, MySQL 4.0.20, and Apache 1.3.29
with mod_php-4.3.4.
