Did you do this per phase in a project, or in the 'phase templates' menu
item? The template is just a template, you need to create projects with
phases. These phases need activities.

Not really. Maybe you can provide 'steps to reproduce'. In other words,
how can I exactly reproduce the problem you have?

You can always edit your own time entries (if you are granted the
privilege in the profile editor). This can be done in the time
registration screen, and in the hour survey (reports menu).
