Thanks for the quick advice.

You aren't going to believe this (well, maybe you will :) but -

 a)  Apache log shows no errors
 b)  PHP logging was disabled (default setting)
 c)  I enabled PHP error logging, and Achievo works fine.
 d)  I disabled PHP error logging again, and Achievo still works fine.

Sigh.  Perhaps I'm going crazy ... 

Now I'm off to explore the software more thoroughly.
