Hi,

I have a problem: I can't add a second employee to achievo. I skimmed
through the mailing list archive, saw that this issue has came up
earlier on this list. There was no patch, workaround or fix for this,
however.

It seems to me to be the same exact problem that Sheridan Chambers ran
into. I ran the addition with $config_debug = 2 and voila:

error: [+00.62169s] write lock on db_sequence failed.
 error: [+00.66333s] Duplicate entry '0' for key 1 (errno: 1062)
 error: [+00.66389s] Invalid SQL: INSERT INTO person
(id,lastname,firstname,address,zipcode,city,state,country,phone,cellular,fax,email,function,remark,role,entity,supervisor,status,userid,password) VALUES ('0', <edited out for brevity>)

No wonder insert whouldn't work. I also checked db_sequences table. It
exists alright, but it's an empty set.

Information on the system:

Vanilla Mandrake Linux 9.1 with:
- apache 2.0.44
- MySQL 4.0.11a
- php 4.3.1

plus achievo 0.9.4 of course.

Workarounds, patches, anyone? Thanks!
