Yes, Achievo uses the db_sequence table to generate unique id's for 
projects. By any chance, does Mandrake to 9.1 perhaps use MySQL 4.0, 
whereas the previous version used MySQL 3?

If that is the case, then you must make sure the database user has the 
global 'lock tables' privilege. In MySQL 3 this wasn't necessary.
