Double check config.inc.php, especially the database stuff.  Then check
your database permissions.  Somewhere along the line you're getting told
you can't use the database.  Remember it's all case sensitive etc.

Btw, I'm assuming you typed that error in by hand otherwise you're going
to get screwed trying to SELECT "form" instead of "from" once you get
past this error.  If you quoted verbatim then try downloading Achievo
again because you've got a broken version.
